Signature Forms


In order for the City of Palo Alto to process the petition in an orderly, consistent and legal manner, petition signatures must be gathered and reported using the EXACT process defined below: .

  1. It is vitally important that the petition signature pages are circulated with the Proposed Ordinance, the Notice of Intent to Circulate Initiative Petition, and one or more Signature Pages STAPLED TOGETHER IN ADVANCE. 
  2. The Declaration of Circulator (at the bottom of the Signature Page) should only be signed by the collector of the signatures AFTER all of the signatures have been collected on that page so you can attest to the validity of those signatures after their receipt - not before. (if multiple signature pages were attached, each Signature Page requires the Declaration of Circulator to be signed). 

Petition signers must have the proposed ordinance and notice right there and attached when they sign. Therefore, all three forms MUST be stapled together before gathering signatures AND MUST remain stapled together when submitted to the city. Please follow this process exactly so that all the hard work of collecting the signatures proves fruitful to the end result. Failure to adhere to this process will likely result in a challenge of the signatures collected.
